Introduction: Pemphigus vulgaris is an autoimmune blistering disease of the skin and mucous membrane. Antiphospholipid antibodies are a group of autoantibodies found in patients with autoimmune diseases. Due to the risk for thrombotic events in autoimmune diseases, we decided to evaluate the correlation between pemphigus vulgaris and predictive elements of atherothrombosis including antiphospholipid antibodies and homocysteine. Methods: IgG and IgM anticardiolipin antibodies, IgG anticardiolipinbeta 2 glycoprotein I, lupus anticoagulant and total homocysteine were evaluated in the serum of 39 new cases of pemphigus vulgaris and healthy matched controls. Results: The prevalence of antiphospholipid antibodies, lupus anticoagulant and total homocysteine in pemphigus vulgaris patients showed no significant difference with healthy controls. Conclusion: Pemphigus vulgaris seems not to be similar to some other organ specific autoimmune diseases in which there is an increase in atherothrombotic factors including antiphospholipid antibodies and homocysteine.

Background: Basal Cell Carcinoma (BCC) is the most common type of skin cancer in human beings. Photodynamic therapy (PDT) is a novel therapeutic method which may be regarded as a non-invasive useful alternative for traditional treatments of BCC. This study was designed with the aim of evaluating the primary response of BCC to PDT.Methods: This clinical trial was performed on 28 BCC lesions. Patients were treated with ALA-PDT monthly for 1-6 sessions and evaluated for clinical response, cosmetic results and probable side effects.Results: Twenty three out of 28 lesions showed response to PDT (9 showed complete response and 14 showed partial response). The cosmetic result was excellent or good in 77.5% of the cases. In 92.8% of the cases, no or mild side effects were observed. Patients with a positive history of radiotherapy responded less (p<0.05). No significant relationship was found between patients' age, sex, localization, diameter or type of BCC with clinical response.Conclusion: PDT might be a good therapeutic option in the treatment of BCC, especially the superficial type, with high efficacy and few side effects.

Background: Pruritus is one of the most disabling problems in patients with chronic renal failure. It is a distressing symptom with a negative impact on quality of life and is also very frustrating for both patients and their physicians. The pathophysiological mechanisms of pruritus are mainly unknown. The aim of this study was to identify factors associated with pruritus in these patients. Methods: This cross sectional study was done on 100 patients who were under hemodialysis because of end stage renal disease. In patients who were suffering from uremic pruritus, the severity of pruritus was assessed and the relationship between the presence and severity of pruritus with serum levels of hemoglobin, creatinine, urea, phosphate, calcium, albumin, parathormone hormone (PTH) and alkaline phosphatase was evaluated.Results: One hundred patients (48 women and 52 men) entered this study. Pruritus was found in 45% of the patients. We found a significant relationship between severity of pruritus with age and serum level of phosphate. No significant correlation was found between the severity of pruritus and sex, duration of dialysis, underlying renal disease and serum levels of hemoglobolin, urea, albumin, PTH, alkaline phosphatase, calcium, and creatinine. Conclusions: Pruritus is still a common problem in hemodialysis – dependent patients. Higher serum levels of phosphate seems to be an important factor associated with uremic pruritus.

A 12-year-old girl was visited at our clinic with multiple soft tissue and bony painless swellings over her right hand since she was 2 years old (Figure 1). Soft tissue masses increased in size and number. She was born to non-consanguineous parents. Her early development was normal and she had no other medical problems. There was no family history of similar problem. Examination revealed multiple compressible soft tissue masses and hard nodules of varying size over fingers and manus of the right hand with superficial hyperkeratosis. There was no limb asymmetry. Her height and weight were within normal range. Examination of parents was normal. X-ray of the right hand is shown in figure 2. A biopsy specimen was taken from a skin nodule. Background: Cutaneous drug reaction is a common side effect of antiepileptic drugs (AEDs). In recent years, a significant increase in cases of cutaneous drug reaction due to AEDs and some changes in its pattern in our department have been noticed. Therefore, we carried out this study to present clinical and paraclinical characteristics of these cases. Methods: All records of patients who were hospitalized at our hospital with a diagnosis of cutaneous drug reaction due to anticonvulsant therapy between March 2006 and September 2009 were reviewed. Results: The most common offending drug was lamotrigine. The main indication of anticonvulsant therapy was for idiopathic seizures. The most frequent type of cutaneous reaction was maculopapular and/or erythrodermic rash. Eosinophilia was detected in 56.5% of the patients. Conclusions: Although serious reactions with AEDs are not common, they may be life threatening. So, timely and accurate diagnosis can prevent fatal reactions and affects subsequent anticonvulsant treatment options.

Granuloma faciale (GF) is a rare cutaneous disorder categorized as a localized form of small-vessel vasculitis. Clinically, it manifests as single or multiple well-demarcated red-brown plaques, papules and nodules, nearly always confined to the face. Herein, we report a 39-year-old man with multiple red-brown infiltrated plaques on face and extrafacial lesions on back, shoulders and both arms. Skin biopsy revealed typical histopathological findings of GF. The patient failed to respond to pulsed dye laser but intralesional triamcinolone combined with cryotherapy led to an acceptable response.
