Background: Acne vulgaris is the most common disorder of pilosebaceous unit. Systemic antibiotics are known to be effective in its treatment. We performed this investigation to compare the efficacy of azithromycin with doxycycline in the treatment of acne vulgaris. Method: A twelve-week study was performed on 69 patients with moderate facial acne to compare the efficacy of oral azithromycin with oral doxycycline. Sixty patients completed the study. Patients in the treatment arm one were scheduled to receive 500 mg azithromycin once daily three times a week, and patients in the treatment arm two were instructed to use 100 mg doxycycline daily. All patients administered topical tretinoin cream every other night. Clinical assessment was made at baseline and then every 4 weeks. Result: There were statistically significant improvements in comedones and inflammatory lesion counts in both groups. Neither drug was shown to be more effective than the other. Conclusion: This study indicated that azithromycin had similar efficacy to doxycycline in reducing acne lesions.

Cutaneous pseudolymphoma (PL) refers to a reactive T or B- cell proliferative disorder in reaction to some known or unknown stimuli. The most common site of involvement of pseudolymphoma is the face followed by the scalp. Because of its similarity with true lymphoma in both clinical and pathological aspects, differentiating between them is often difficult but fundamental due to their completely different prognoses and treatments. We report a case of cutaneous pseudolymphoma of the breast in association with fibrocystic change of the breast, and a long history of taking hormonal drugs, in a 35-year-old woman. There was a very good response to intralesional steroid injections and oral hydroxychloroquine.

Background: Few studies have investigated on vitamin D metabolites Serum levels, and calcium homeostasis in humans receiving retinoids, despite a substantial amount of literature concerning retinoid-induced osteoporosis in animals. We prospectively measured vitamin D metabolites serum levels and calcium homeostasis and radiographic bone changes in short course treatment with oral isotretinoin in severe acne. Methods: 1,25-dihydroxy vitamin D and 25-hydroxy vitamin D, calcium, phosphate, Parathyroid hormone and axial spine , femoral neck radiographies were measured in 30 nodulocystic acne patients ( 17 -28 years ) before and after twenty weeks of treatment with oral isotretinoin at the recommended dose (0.75 -1 mg/kg/day) .Results: vitamin D metabolites, calcium, phosphate, Parathyroid-hormone did not change significantly and hyperostosis and other sclerotic change were not observed in any patients. Conclusion: Short course treatment with oral isotretinoin at the recommended dose did not lead to any significant changes in serum vitamin D metabolites, calcium, phosphate and Parathyroid hormone and has no effects on bone radiography in acne patients.

Lupoid leishmaniasis is an unusual complication of acute cutaneous leishmaniasis which can represent a diagnostic and therapeutic challenge. The disease is actually a reactivation of an earlier acute cutaneous infection. It usually starts close to or in the scar of previous lesion and can cause severe destruction. Its diagnosis is complicated by the sparsity of the microorganisms in the direct smear and the tissue. The present report deals with a middle age Iranian woman presented to us with multiple scars on the whole face and also multiple red-down papules and nodules around these scars. After three years of clinical, histopathological and laboratory investigations and treating with no or minimal success for the diagnosed of discoid lupus erythematosus, sarcoidosis and lupus vulgaris, finally was diagnosed and treated successfully as lupoid leishmaniasis with meglumine antimoniate (Glucantime).

Linear psoriasis is a rare entity which is commonly confused with inflammatory linear verrucous epidermal nevus. We report a 25-year-old man who had linear psoriasis along Blaschko lines in a unilateral distribution on the left side of the body. Our diagnosis was based on a combination of historical, clinical, histopathological evidence and response to treatment.
