Helicobacter Pylori and skin: Any relationship?

Helicobacter pylori is probably the commonest bacterial infection worldwide and is now accepted as the cause of gastritis, peptic ulcer, gastric carcinoma, and gastric lymphoma. In the last few years, besides the local gasteroduodenal tissue damage, an association between H.pylori and various extra-intestinal pathologies had been described. Recent studies have suggested that H.pylori infection may be associated with various skin disorders such as idiopathic chronic urticaria, Raynaud’s phenomenon, systemic sclerosis, rosacea, Sweet’s syndrome, Henoch-Schonlein purpura, psoriasis, and atopic dermatitis. In this paper we review the current knowledge on Helicobacter pylori infection and its relevance for skin diseases especially its clinical pathophysiological aspects.

Prevalence of nail disorders in Tehran elementary schools

Nails can act as a protective, defensive and beauty cover of the phalanges. The dermatologic disorders as well as the systemic diseases and trauma can produce change in the nails. Normal variations of nails are important to recognize and distinguish from nail diseases. There is not enough information about the prevalence of nail disorders in children. In this study, we have determined the prevalence of leukonychia, pitting and koilonychia in Tehran elementary schools. Examinations were conducted among 1902 elementary students and leukonychia in 16.9%, pitting in 2.1% and koilonychia in 1.8% were found. There was no difference in the overall frequency of the nail disorders between males and females. The prevalence of leukonychia and pitting was more frequent in the 5th grade than first grade and in 11th educational area of the education ministry. There was not any important relation between the nail disorders and dermatologic disorders or systemic diseases, trauma may be the most important factor in these nail disorders. As regards to the importance of diagnosing the hypochromic anemia and cystine deficiency, we suggest a scientific research about koilonychia and those disorders.

Alopecia Areata and Dermatoglyphics

Objective: To study of relationship between alopecia areata and finger print dermatoglyphic types in 100 patients with alopecia areata and in 100 controls. Methods: A case control analytical study was done on fingerprints of patients with alopecia areata. The control cases were older than 50 years old without any history of alopecia areata. Dermatoglyphical trials of total fingers were also studied. Diagnostic criteria for alopecia areata was simultaneous shedding of hair seen by a dermatologist. Results: In patients with alopecia areata, the frequency of finger print types was: whorl in 47.3%, loop in 39.6%, arch in 13.1%. In control group whorl were seen in 56.1%, loop in 37.3% and arch in 6.6%. The statistical difference between fingerprint of thumb finger in right hand, index and small finger in left hand relative to control group were significant (P<0.05). Conclusion: The findings indicate of a genetic relation between alopecia areata and the finger print dermatoglyphics.

Hypnotherapy in the treatment of recalcitrant Warts

Hypnotherapy is recommended for recalcitrant cases of many dermatologic disorders including viral warts, chronic urticarias, psoriasis and alopecia areata. A 23-year old, married lady referred with five years history of confluent verrucous and cauliflower type of viral warts on the dorsum of right foot and many discrete warty papules on the left foot and toes and on different aspects of both hands and fingers. She had tried many medical modalities and surgical procedures including: keratolytics, formalin application, electrocauterization, curettage, cryotherapy with no satisfactory effect. As the patient was quite frustrated with no more trust on physical therapies, she was advised to be treated by hypnotherapy. After patient’s consent, she was scheduled for 20 hypnotherapy sessions with weekly interval. The papules on the hands disappeared after the second session and those of the right foot started to improve after the tenth session. Complete eradication occurred before the 20th session. Although hypnotherapy is time consuming, it is recommended for recalcitrant cases of viral warts.

Trichoadenoma: A case report

Trichoadenoma is a rare skin tumor first described by Nikolowski. There are variable clinical features and tumors have been diagnosed as cysts, basal cell epithelioma and seborrheic keratosis. The microscopic feature mainly consists of numerous cysts surrounded by eosinophilic cells. Trichoadenoma is benign and treatment is by simple excision. We report a case of trichoadenoma on the nose of a 46-year-old man present for 2 years.

A 9-year old boy had severe muscle weakness and typical skin rash and EMG with diagnosis of dermatomyositis associated with erythrodermia with islands of normal skin and palmoplantar hyperkeratosis, which was reported. As PRP in skin biopsy. Association dermatomyositis with PRP is very rare.

Diffuse plane Xanthomatosis: A case report

A 58-year-old man had 3 years history of asymptomatic, extensive, yellowish patches on his face, neck, trunk and arms. Histopathological examination confirmed the diagnosis of diffuse plane xanthomatosis. This is a rare non-lipemic disease in which xanthomatous lesions develop in the skin in association with paraproteinemia. Serum lipids are usually normal. The histological features include both xanthomatous and inflammatory elements. The treatment of this condition is that of the underlying myeloproliferative disease or paraproteinemia.
