Background: A high percentage of the patients referred to dermatologic clinics are either suffer from self inflicted disorders or from misinterpretation about their own health and organ integrity and or may suffer from different skin sensations including itching, pain, and burning; all of which may develop as a result of their underlying psychopathologies. Objective: Identification of underlying psychopathology of psychocutaneous disorders. Patients and Methods: 109 dermatologic patients who clinically diagnosed as psychocutaneous disorders referred to psychiatric clinic for identification of their mood, personality and thought disorders. The impression of psychocutaneous disorder was made according to clinical interview and physical examinations in dermatology office. A semi-structured interview based on DSM-IV criteria performed on all patients to evaluate their mood, personality and thought. They also filled the Beck’s and Tailor inventories for further evaluation of their possible depression and anxiety. Results: 107 out of 109 cases studied, suffered from mood disorders including anxiety, depression or a combination of them. Of the remaining two cases one suffered from attention deficit hyperactivity disorder (ADHD) and the second one had no psychiatric disorder. 17 out of these 107 cases also suffered from personality disorder and 3 cases had delusional thoughts. Conclusion: All psychocutaneous patients may suffer from one or more psychiatric disorders which should be identified and approached properly.

Lateral labial melanocytic nevus or mole has cosmetic value among young or middle aged Iranian ladies particularly those with fair complexion. I describe auto transplantation of a melanocytic nevus from a remote area of the body to lateral labial skin. After selection of a 3-mm nevus, both donor and recipient sites were anesthetized. A punch with a diameter comparable to the diameter of the nevus was chosen. First the recipient site was punched out and discarded. Then the nevus was punched out and transplanted in the recipient site and dressed by dry gauze and fixed by adhesive tape. The transplanted nevus was attached to surrounding skin after 10 years. A superficial crust was developed and seen after removing the dressing 10 days later. Mild shrinkage in size and reduction in pigmentation was seen a month later, which gradually improved thereafter.

Background: Paederus dermatitis, an acute erythemato-vesiculo-bullous dermatitis contracted through contact with the secretions of the Paederus species, is frequently seen among the people living in the south-west part of our country during spring and early summer months. Objective: This study was conducted to elucidate the responsible arthropod in Khuzestan province and to study the clinical spectrum as well as the pathological findings at different stages of the disease. Patients and Methods: Within a period of about 3 months, from April to July 1999, 72 patients referred to our clinic with erythema, vesicles, bullae, pustules, superficial skin necrosis or combination of few or all of these findings clinically suggestive of Paederus dermatitis. Biopsy samples were obtained from 19 patients at different stages of the disease stained by HE and studied histologically. All arthropods submitted by the patients and their families were crushed on our skin to identify the causative species. Results: The dermatosis is contracted through contact with the secretions of a Coleoptera species called Paederus Ramtinus. The painless and non-pruritic reaction developed 2-3 days following contact as erythema and vesiculation, which may gradually extend to necrosis and bullae. The pathologic changes were also variable and ranged from epidermal spongiosis to complete epidermal and upper dermal necrosis. Conclusion: Paederus dermatosis developed as a result of contact with the secretions of the species Paederus Ramtinus if by any way crushed or brushed off over the skin. The dermatosis is benign in contrast to its acute course no sequela is seen except for post inflammatory hyperpigmentation.

Background: Psoriasis is a common and genetically determined skin disease, which is frequent throughout the world. Objective: To perform a descriptive clinical study of 200 cases of psoriasis in Ahwaz. Patients and Methods: This study was performed from November 1997 to April 1999 on 200 psoriatic patients attended a private skin clinic in Ahwaz. Data were collected from patients and entered in questionnaires. Results: 56% of the patients were female and 44% were male. The age of onset was in the 1st, 2nd, 3rd and 4th decades in 24.2%, 33.5%, 18.5% and 10%, respectively. Their age was in the 1st, 2nd, 3rd and 4th decades in 23.5%, 28.5%, 17.5% and 14%, respectively. Only few cases referred with later ages of onset. In 40% of the cases the disease started from the scalp. 9% of the patients reported positive Koebner phenomenon. A positive family history for psoriasis was found in 25%. Nail involvement was detected in 39% of the patients. Conclusion: Psoriasis in Ahwaz shows a similar pattern to the other parts of world.

Apocrine nevus is a rare benign tumor of which only 9 cases have been reported in the literature. The apocrine nevus is seen mostly in the areas where the apocrine glands are normally distributed such as axillae, head and neck. Our case was a 16-year old girl who referred with a verrucous erythematous plaque over right parietal side of the scalp. The patient had the plaque since birth with partial growth after puberty. Histopathologic study showed mature apocrine glands located in the reticular and papillary dermis with epidermal papillomatosis and acanthosis.

Background: Thirty to forty percent of the patients referred to dermatologic clinics are estimated to have psychiatric problems. These patients either are those who have developed dermatologic problems as a consequence of their primary psychiatric disorders or those who have developed psychiatric disorders following their chronic and disfiguring dermatologic problems. The third group are psychophysiologic disorders which may aggravate following psychiatric or emotional stress. Objectives: Our purpose is to stress the significance of the psychodermatologic problems for both dermatologists and psychiatrists; to have an overview and a brief classification of these and discussing the approach to the patients with psychodermatologic disorders. Methods: The dermatologists and psychiatrists’ pitfalls in approaching the psychodermatologic disorders are discussed. Psychodermatologic disorders are classified and their underlying psychopathologies are mentioned. Conclusion: Both dermatologists and psychiatrists should be trained more to manage these relatively frequent patients well; for this purpose establishing the dermatology-psychiatry liaison clinics and or helding the dermatologic-psychiatric conferences are advised.
