Background and aim: Psoriasis is one of the most common papulosquamous diseases that often requires long-term maintenance therapy. Long-term use of topical corticosteroids on the face and intertriginous areas will increase the risk of steroids side effects such as striae and atrophy development. A safe, effective and inexpensive modality in topical therapy of inverse psoriasis is required. adrenergic antagonists has been reported to exacerbate existing plaque psoriasis because of increasing concentration of intracellular cAMP. On the other hand, caffeine is a methylxanthin that inhibits phosphodiesterase enzyme and results in a higher concentration of intracellular cAMP. We evaluated the efficacy of topical caffeine 10% in the treatment of psoriasis.Materials and Methods: This randomized, double blind, controlled clinical trial was performed on 39 patients with typical lesions of psoriasis vulgaris who referred to dermatology clinics of Isfahan University of Medical Sciences from December 2002 to July 2003. These patients were randomized in two groups and treated by topical application of drug or placebo, three times per day. The patients were seen in two-week intervals for 8 weeks, and their PASI score was assessed in each visit.Results: The decrease of PASI score after 2,4,6 and 8 weeks compared to the first visit in caffeine treated group were 23%, 41%, 54%, and 62% and in placebo treated group were 15%, 19%, 25%, and 29% respectively(P<0.05).Conclusion: Topical caffeine may be an effective, safe and inexpensive therapy in psoriasis but its onset of action is late.

Since the discovery of X-rays, the use of imaging technology has continued to play an important role in medicine. Technological advancements have led to the development of various imaging modalities, most of which have been used to image organs deep within the human body. More recently, attention has focused on the application of imaging technology for evaluation of the skin. A variety of techniques are currently being used to examine the skin and these include specialized photography, surface microscopy, ultrasound, laser Doppler perfusion imaging, confocal microscopy, magnetic resonance imaging and optical coherence tumography. These modalities can provide information hat can assist in the management of skin problems. Although many of these techniques are still undergoing research, they are showing promise as useful clinical tools in dermatology. In this review article we discuss about each of these technologies in brief.

Basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) are the most frequent skin cancers. The etiology of these tumors is multifactorial: Environmental and exogenous factors, genotype and phenotype factors. The development of malignant neoplasms at sites of previous dermal scars is an uncommon but well recognized phenomenon, and since almost all of them were developed on exposed areas it has been hypothesized that the accompanying atrophy and decreased vascularity and adnexal structures in areas of scarring may render the affected tissues more susceptible to the effects of environmental factors, eg. ultraviolet light. We report four cases of BCC and SCC on old leishmania scars on face, and recommend that any changes on scars must be evaluated clinically and pathologically.

Direct smears of the lesions were positive for Leishman bodies. Leishmanin skin test was negative. He also had dactylitis in some digits and toes. PCR from bone biopsy showed Mycobacterium tuberculosis. So he was treated according to diagnosis of disseminated cutaneous leishmaniasis with tuberculous dactylitis.

Pemphigus is a chronic blistering mucocutaneous disease, classified in immunobullous skin diseases. Infections, particularly septicemia due to Staphylococcus aureus and Pseudomonas aeroginosa are the most common and dangerous disorders due to immunosuppressive therapy in this disease. Infection with nocardia is rare, so that only 6 cases have been reported yet. Herein, we report a 56-year-old man with one-year history of pemphigus treated with prednisolone and cyclophosphamide, who at the same time developed skin and pulmonary nocardiosis. He had a good response to treatment with cotrimoxazole.

Linear morphea is a distinct form of morphea that occurs as a linear band usually with a single unilateral distribution. The lower extremities are most often involved. This dermatosis appears as a linear induration not necessarily related to the derm induration. We report herein a case of 20-year-old Iranian girl who had a linear sclerotic patch on right lower extremity with clinical and pathologic diagnosis of linear morphea. Several perforating papules appeared on the lesion after 6 months, confirmed as perforating dermatosis by the biopsy.

Background: Some researchers claim that permethrin-impregnated clothes are effective in the prevention of cutaneous leishmaniasis. Objectives: To determine the effectiveness of permethrin-impregnated clothes in the prevention of cutaneous leishmaniasis. Patients and Methods: 272 soldiers were randomized in two groups. In one group permethrin-impregnated clothes and in the other group, control clothes were used. In each group the clothes were worn during days and nights for three months and the soldiers were followed for additional 6 months. Results: 9 (6.5%) of 138 soldiers wearing control clothes and 6 (4.4%) of 134 soldiers wearing permethrin-impregnated clothes acquired cutaneous leishmaniasis. The difference between two groups was not statistically significant. Conclusion: Permethrin-impregnated clothes are not effective in the prevention of cutaneous leishmaniasis.

Background: Cutaneous leishmaniasis (CL) is a parasitic disease caused by leishmania species. An ideal treatment for CL has not found yet. Objectives: To evaluate the efficacy of cryotherapy alone and combined with intralesional glucantime injection in the treatment of papulonodular CL. Patients and Methods: 300 patients with CL were randomly assigned to two treatment groups in this clinical trial. In group 1, 100 patients with 149 lesions were treated with cryotherapy plus intralesional glucantime injection every 2 weeks. In group 2, 200 patients with 230 lesions were treated only with cryotherapy, every 2 weeks. Both groups were followed for 6 months after last treatment. Results: Clinical and parasitological cure were seen in 90% of cases in group 1 and 57.3% in group 2 (P<0.05). Conclusion: Combination of cryotherapy and intralesional glucantime injection is an effective treatment modality in early lesions of CL.

Background: Acitretin therapy is frequently associated with reversible, dose-related side effects. Recent studies claimed that combining vitamin E with high-dose isotretinoin reduced isotretinoin-induced side effects. Objective: The purpose of this clinical trial study was to determine the effect of a fixed or adjusted dose of vitamin E on the side effects of acitretin. Patients and Methods: Fifty five subjects were randomly assigned to one of the two treatment programs including with acitretin (0.7-1 mg/kg/day) together with either vitamin E (13 IU/kg/day) or alone for 3 months. The incidence of side effects of acitretin in two groups were assessed and compared. Results: The study was completed in thirty-nine patients. Vitamin E did not decreased the incidence of side effects associated with acitretin. Conclusion: Vitamin E did not significantly ameliorate retinoid side affects when combined with 0.7-1 mg/kg of acitretin in treatment of skin disorders.

Background: Pain and pruritus are two common symptoms in dermatology that are transmitted with unmyelinated C fibers via substance P. Topical capsaicin is effective due to depletion of never endings from substance P. Objective: The efficacy and side effects capsaicin gel in painful and pruritic skin disorders was compared to placebo. Patients and Methods: In a controlled clinical trial, 40 patients (21 patients with post-herpetic neuralgia, 10 with lichen simplex chronicus, and 9 with notalgia paresthetica), with the mean age of 48 years were randomly treated with 0.075% capsaicin gel or placebo q.i.d. for 4 weeks. The severity of symptoms and side effects were assessed every two weeks and recurrence was assessed also two months after treatment. Results: 31 cases (77%) completed their 4 weeks treatment period. According to visual Analogue Score system, the mean of symptom severity decreased from 7.6 (±2.4) before treatment to 0.9 (±0.9) after treatment in capsaicin group and decreased from 6.3 (±2.3) to 5.0 (±3.2) in placebo group (P<0.05). Conclusion: Capsaicin gel is effective in painful and pruritic skin disorders and has tolerable side effects.

But inflammatory epidermal nevus has distinct histopathologic features. We report a case of a 12-year-old Iranian girl who had a huge verrucous lesion on left lower limb since age of 4 years. Bone age of the patient was 6 years and her left lower limb was 6 centimeters longer than right side. Her lesion was excised in several sessions and grafted but showed recurrence.

Background: Nevus spilus can produce considerable disfigurement. There is no specific and successful treatment for this nevus. Objective: The purpose of this study was to evaluate the efficacy of CO2 laser in the treatment of nevus spilus. Patients and Methods: This quasi-experimental clinical trial was performed on 47 patients with spilus nevus in Al-Zahra Hospital in Isfahan. The diagnosis of nevus spilus was based an clinical and biopsy findings. The efficacy of treatment was evaluated based on tolerability, side effects, improvement according to patient and physician judgment and comparison of photographies, after one year of follow up. The laser used was Sonic LS 500 (Made in Germany) with power of 3 watts, duration of 0.25 seconds and spot size of 3 millimeters. Results: 45 of the patients were female and 2 were male. The mean age of them was 20 years. All the lesions were on the face. Two patients were withdrawn due to inefficacy or infection of test site treatment. In 43 of the 45 remaining patients (95.5%), the treated site healed uneventfully. Recurrence of nevus spilus was seen in a single patient one year after treatment. Conclusion: CO2 laser is an effective therapy for nevus spilus.

Introduction: Macular amyloidosis is a relatively rare disease. A basic change appears to be amyloid material deposition in an apparently normal skin. The characteristic features of the disease are reticulate and ripple brown pigmentation in interscapular regions. Shoulder, back and chest. There are several reports indicating the association of the disease with rubbing against the skin. According to high frequency of the disease in our area especially among the women and due to its disability such as disfigurement for women, we decided to perform the following study. Materials and Methods: The study is an analytic descriptive one. Twenty-eight patients suffering from macular amyloidosis defined by clinical and laboratory findings were reviewed. The study being performed from October 1995 to October 1998 in University hospital dermatological clinics in Isfahan. Results: Twenty-one females and seven males were considered in the study. The age range was between 15-50 years. There was a positive family history in 43% of patients. Clinically the lesions were composed of reticulate and rippled pigmentation frequently observed in interscapular, upper arms and shoulder regions. The most common association in these patients was seborrheic dermatitis of scalp and face. In this study most of the patients mentioned the use of nylon brush during bathing and some of them relieved their itching with comb and brush. Conclusion: Female to male ratio in our study was three to one, which is compatible with higher frequency of disease among women in recent reports. The age range of patients was frequently between 15-34 years that is in young adults. A direct relationship was present between nylon brush or comb use during bathing and exacerbation of the disease. The traditional habit of using nylon objects during bathing might explain the high prevalence of this disorder in our community.

Explosion of medical information, increase in the speed of communications and the pressure of WHO on observing scientific rules, oblige dermatologists to have a good education and to use more effective education principles. A brief review on the publications on learning principles reveals only a small role for dermatologists. The minimum benefits of these principles are to acquire a scientific basis to found a suitable pattern for continuing education for us and for teaching our students. Learning and teaching skills can be improved by scientific education. Successful teaching will result in a positive feedback encouraging both teachers and residents. This review will mostly emphasize on learning skills.

Hypersensitivity to anticonvulsant drugs have been reported many times. But anticonvulsant hypersensitivity syndrome (AHS) is a potentially fatal drug reaction with cutaneous and systemic reaction to the arene oxide-producing anticonvulsants, phenytoin, carbamazepine, and Phenobarbital sodium. The hall-mark features of this syndrome are: Fever, rash and lymphadenopathy. The epoxide hydrolase enzyme may be lacking or mutated in persons in whom AHS develops. The reaction may be genetically determined and familial occurrence of hypersensitivity was observed. The timely recognition of AHS is important, because accurate diagnosis avoids potentially fatal re-exposure and affects subsequent anticonvulsant treatment options. We report two cases of AHS and review the clinical and pathophysiologic features.
