Background and aim: Acne is a chronic inflammatory disease of the pilosebaceous units. Its etiology is unknown. However, several studies have shown that some endocrinologic factors are involved in acne pathogenesis. The aim of this study was to assess some endocrionlogic indices in male patients with acne vulgaris and comparing them with normal controls.Materials and Methods: In this case-control study, 40 male patients with acne referred to Razi hospital during the first 9 months of 1382, and 20 age-matched healthy subjects were selected. Plasma level of insulin, fasting blood sugar (FBS), free testosterone and sex hormone binding globulin (SHBG) were determined and compared between the groups.Results: The mean FBS level was 89.8±13.3 mg/dl in patients with acne and 91.1±16.3 in controls (P=0.017). The mean free testosterone level in cases and controls were 24.8±12.01 and 21.4±10.8 nmol/L, respectively (P=0.029).Conclusion: In this study, the testosterone level in patients with acne was higher and the FBS level was lower than controls. It is recommended that these indices be taken into consideration in male patients with acne.

Nowadays, Erbium: YAG laser resurfacing of periorbital skin has become a very popular method of wrinkle reduction. Seven patients with Fitzpatrick skin phototypes of II to V were treated with a 2940 nm Erbium: YAG laser system for resurfacing of periorbital wrinkles. Photographic evalution performed before and after treatment. Patients were visited at 1,2,3,5,7,14,30 and 60 days after laser resurfacing. The results were graded into five categories: no improvement (grade 0), mild (grade 1: up to 25% ), moderate (grade 2: 25-50%), good (grade 3: 50-75%) or excellent (grade 4: 75-100%). Two independent investigators and also the patients themselves evaluated the improvement grades by comparing the photographs. Peri- orbital skin re- epithelization after treatment took a mean duration of 3.7 days. Duration of erythema varied from two to seven weeks with a mean duration of 16.2 days. Eight weeks after the end of the treatment the mean improvement grade was 2.35 (moderate to good) by investigators judgment and 2.14 (moderate to good) by the patients themselves. No pigmentary complications were evident after 8 weeks. No scars or ectropion were observed.

Immunosuppression is known to be associated with increased rate of malignancies and widespread dermatophytosis in the patients with sezary syndrome and this may account for the occurrence of Kaposi’s sarcoma and generalized dermatophytosis in patients in this report. Herein, we report a 58-year-old farmer man, who admitted with two months history of severely pruritic skin lesions, which progressed to erythroderma. Concurrently some vascular lesions were observed on glans penis off the patient suggestive of Kaposi’s sarcoma. Leukocytosis with eosinophilia (20%) and atypical lymphocytosis (More than 10%) were present. Microscopic examination of biopsy of erythrodermic skin lesions on trunk revealed pautrier micro abscess with dermal lymphocytic infiltration and cellular atypia, and the peripheral blood sample showing sezary cells and high ratio of CD4/CD8 (45) lymphocyte were in favor of diagnosis of sezary syndrome. The pathologic evaluation of the vascular lesion of glans penis proved the diagnosis of Kaposi’s sarcoma. During the progression of disease, the patient was invaded by generalized dermatophyte infection. KOH smear of annular skin lesion showed mycelia of dermatophyte and culture proved trichophyton rubrum to be the pathogenic agent.

Background: Psoriatic arthropathy is a seronegative arthropathy seen in some patients with psoriasis, a self-limited and genetically determined skin disease. Objective: To determine the prevalence and risk factors of arthropathy in patients with psoriasis. Patients and Methods: In a prospective, cross-sectional study, psoriatic patients referred to Razi Skin Hospital in Tehran, Iran in 2002 were evaluated for the presence of arthropathy, and were referred to a rheumatologist for confirmation of arthropathy. The relationship between and nail involvement were evaluated using chi-square and t tests. Results: Among 320 patients with psoriasis, 29 (9.1%) had joint diseases. The most common was asymmetric polyarthritis. Patients with joint diseases had more nail involvement (96.5% vs. 73.1%, P<0.05) and higher mean PASI scores (24.33±10.35 vs. 10.70±8.44, P<0.05) than those without arthropathy, but the age and sex distribution of two groups were not significantly different. Conclusion: The prevalence of joint diseases in psoriatic patients in this study was similar to other reports. Nail involvement and more extensive disease are risk factors for joint diseases in these patients.

Background: Due to the chronic nature of atopic dermatitis and dependence of most patients to steroids, finding a suitable alternative treatment is important. Objectives: To determine the efficacy of phototherapy in treatment of atopic dermatitis, also to evaluate possible influencing factors in response of patients to psoralen+UVA (PUVA) therapy. Patients and Methods: 13 patients with resistant atopic dermatitis, older than 10 years, were treated with PUVA, 2 or 3 times a week until complete recovery or maximum of 30 sessions. Rate of recovery was determined according to reduction in 6 signs including itching, erythema, lichenification, xerosis, exfoliation and excoriation at the end of the treatment period and 2 months later. Results: 10 patients (77%) at the end of the treatment and 9 patients (69.2%) two months after that, showed more than 50% recovery. There was no significant relationship between the pretreatment serum IgE levels, site of the lesions, age, sex and phototherapy. The patients with family history of atopic dermatitis showed a better response to the treatment. Conclusion: PUVA is a suitable alternative treatment in resistant cases of atopic dermatitis.

Background: Hirsutism is a common clinical problem with different etiologic factors. Hence, clinical and laboratory studies are necessary to find these factors. Objectives: To determine the etiologic factors of hirsutism with clinical and paraclinical studies including TSH, DHEA-S, LH/FSH, testosterone, prolactin, 17-OH progesterone and sonography of ovary. Patients and Methods: This cross sectional study evaluated clinical and paraclinical findings of 110 hirsute patients who referred to the dermatology clinic of Tehran Razi Hospital during a period of one and half years, in 2000-2001. Results: Polycystic ovary syndrome (PCOs) was diagnosed in 54 patients (49%) including 14 cases (26%) with regular menstruation. 43 patients (39%) had laboratory findings compatible with PCOs and LH/FSH>3 was the most valuable one in 41 patients (37%). 53 patients (48%) with regular menstrual cycles and normal laboratory and sonographic findings had idiopathic hirsutism. Late onset adrenal hyperplasia due to partial 21-hydroxylase deficiency was diagnosed in two cases (1.8%). In one patient with irregular menstruation, pituitary prolactinoma was confirmed by brain imaging techniques. Conclusion: Routine laboratory evaluations are mandatory in the management of hirsutism, regardless of menstruation cycles.
