Acne keloidalis is a chronic inflammatory process that involves hair follicles of the nape of the neck and leads to hypertrophic scarring. In all references, it is noticed that this process occurs only in males after puberty and is especially seen in the black race. In this case, we report a 31-year-old white woman who had hypertrophic papules in the nape of her neck since 10 years ago and in pathological examination, the diagnosis of acne keloidalis was confirmed.

Nevus comedonicus is an uncommon variant of adnexal hamartoma and is considered a rare subtype of epidermal nevi. It was first described in 1895 by Kofmann who used the term “comedo-nevus”. It manifests as a group of closely dilated follicular openings with dark keratin plugs resembling comedones. Both unilateral and bilateral distributions are seen. The face is the most commonly affected site followed by the neck, the trunk, and the upper arms. We report a case of a 25-year-old male who presented with linear keratotic papules which on histopathology was confirmed to be nevus comedonicus. Our case was interesting because of its large size and of its multidermatomal involvement affecting the chest, back, and arms.

case: A 3.5-year-old girl presented with a 1-year history of a slow growing pigmented lesion on the dorsal aspect of her right foot. Physical examination revealed the presence of a pigmented patch with color distribution from pink to tan to dark brown, relatively well circumscribed and approximately 1.5 × 2cm with irregular borders, and especially a single dark central papule superimposed in the middle of it (Figure 1). The lesion had a soft consistency and the child did not have any other symptoms. During the last year, it had increased in dimensions and thickness. The remainder of her physical examination was insignificant. Also, there was no positive family history of a particular disease or any history of trauma or drug usage. A 3 mm punch biopsy was obtained from the central dark papule and the specimen was sent for histopathologic examination.

We report a 61-year-old man with multiple colored skin papules with central crusts in an archiform and linear pattern on his forehead as well as verrucus and hyperkeratotic plaques with central perforation on hands. Histologic examination of the lesions demonstrated a narrow channel of epidermal perforation containing notrophils, basophilic debris and elastic fibers with eosinophilic degeneration compatible with the diagnosis of elastosis perforans serpiginosa.

A 21-year-old man was referred to dermatology clinic for skin biopsy. He presented to neurology center with behavioral changes with cognition and memory disorders. He had two episodes of genealized tonic clonic seizure and series of myoclonc jerks in his limbs and face especially after awakening since several months ago. In neurological examination, he had mini mental score as 13; with some degree of bradykinesia, limb rigidity and dysartheria. He had leaved school because of inability to learning. His sister had died 5 months ago from the same disease. We performed biopsy from his right axillary skin. Histopathological examination revealed multiple intracytoplasmic homogenous pale basophilic globi in apocrine glands acini which were showing PAS positive reactivity in special staining. Reminder of epidermis and mid reticular dermis was unremarkable.

A 33–year-old woman was visited at the dermatological clinic of Loghman-e-Hakim Hospital with multiple hypopigmented patches on her right arm, buttocks and flanks. Her lesions developed over the past year with no regression. Lesions were resistant to multiple antifungal therapies. Clinical examination revealed multiple circular or oval patches on the right arm, flank and sacral area. There was no erythema but fine scales were observed on the lesions (Figure1). No cutaneous sensory deficit was detected and no abnormality was found on physical examination. Also, there was no family history of similar problems. Abdominal ultrasonography and chest x-ray were normal. We performed biopsy from one of the lesions.

For many yearshydroquinone was the foundation of melasmatreatment, but its use has been limited during theseyears due to a number of adverse events, suchas skin irritation, hyper and hypo pigmentation,nail discoloration, conjunctival melanosis andochronosis. Furthermore hydroquinone is quiteunstable and is quickly oxidized and becomeinactive. So Hydroquinone is gradually beingreplaced by new lightning substances, such as kojicacid, phytic acid, azaleic acid, arbutin, mequinol,aleosin, ascorbic acid, soya proteins, N-acetylglucosamine and licorice. In this study weevaluate the effect of a licorice containing product(lightening serum, LANGE Paris) for the treatmentof melasma. There are several active ingredientsin licorice extract effective on melasma. These areliquiritin, glabridin, licochalcone A and isoliquertin.

Background: Cutaneous granulomatous dermatoses usually are a diagnostic challenge with tuberculosis (TB) on top of them. TB is a universal health problem and considered by WHO at the top of 6 common and dangerous infectious diseases. Objective: To determine the clinical, laboratory and histopathological findings in 45 patients with the diagnosis of cutaneous TB over a 10-year period in Ahwaz. Patients and Methods: This is an existing data study on the records of all patients with the diagnosis of TB admitted to Sina hospital in Ahwaz from November 1990 to March 1999. Data on the age, localization, clinical features were extracted from hospital records. Results: The age range of patients was between 1-76 years. Twenty-eight cases were female and 17 cases were male. Twenty-three cases were clinically diagnosed as scrofuloderma (SFD), 18 cases as lupus vulgaris (LV), 1 case as erythema induratum, 1 case as TB gumma, 1 case as orificial TB and 1 case as populonecrotic tuberculide. The most common involved site was face and the least common sites of involvement were buttock and elbow. Six cases had positive family history. SFD was present in 12 females and 11 males. The result of tuberculin test was positive in 15, negative in 17 and not available in 13 patients. Histopathologically, 23 cases showed chronic granulomatous disease without caseation, 18 cases showed non-specific inflammation and in 3 cases tubercle with caseation was observed. Concerning treatment, we found generally good clinical response with no failure in 32 patients who came for follow up. Conclusions: This study showed that in Ahwaz SFD was the most common form of cutaneous TB and the most frequent site of involvement was face.

Background: Basal cell carcinoma (BCC) is the most common type of skin malignancy. Hyperthermia is used as adjuvant therapy together with radiotherapy and chemotherapy in the treatment of some cancers. This study aimed to evaluate the efficacy and safety of thermotherapy in the treatment of BCCs.
Methods: This pilot trial was carried out on five patients with nodular or superficial BCCs on the scalp and face.Thermotherapy was done at 50° C for a duration of 30 seconds with a radiofrequency device (Thermomed 1.8 thermosurgery technologies). Thermotherapy treatment was applied only in one session. The first follow-up was one week after the procedure and then every month for six months. The last follow-up was one year after the procedure.
Results: All patients showed clearance of the tumors. Blisters, ulcers, crusts, and necrosis were the side effects, which healed with acceptable degrees of scarring. No recurrence was observed at the end of the study.
Conclusion: Thermotherapy was safe and efficacious for the treatment of BCCs on the scalp and face. It should be considered especially for the low-risk subtypes of this cancer.

A 10-year-old girl presented with a 1-year history of an eruption on her hands. Physical examination revealed soft, bluish slightly tender subcutaneous nodules and swellings in the ulnar site and palmar aspect of five fingers of the right hand and dorsal aspect of three fingers of the left hand without any gross deformity in fingers and long bones (figure 1). History of pathological fractures was negative. X ray of the right hand showed reduction in bone density with lytic lesions in five metacarps and proximal phalanx of the fourth and fifth fingers associated with soft tissue swellings (figure 2). A biopsy specimen was taken from a skin nodule (figure 3).

Lichenoid and granulomatous dermatitis is a new entity which is histologically defined by a band like lymphocytic infiltration in the upper portion of the dermis with obscuring the dermoepidermal junction and accompanied by basal layer degeneration and granulomatous inflammation. Herein, we presented two Iranian women with similar cutaneous and histopathologic findings. Both patients had asymptomatic subcutaneous nodules on their hands. The pathological manifestation in both of them was lichenoid and granulomatous dermatitis. Our investigations indicated different underlying causes of the lesions in our patients. In one patient, the lesion was associated with hyperthyroidism and in the other one it was accompanied by rheumatoid arthritis.

Case: A 54-year-old woman was visited with a history of asymptomatic gray-brown discoloration of the facial skin at our dermatology clinic. The lesions first appeared on her chin and then became progressively darker and extended to her nose and, to a lesser extent, to the periphery of her face over a period of five years. She mentioned that the lesions worsened with heat and sun exposure. Her past medical history was unremarkable. The patient’s medications included sunscreens, and hydroquinone lightening creams from many years ago. Physical examination revealed a previous depressed scar of cutaneous leishmaniasis on her forehead, brown to black hyperpigmented macules and patches, more in her midface area, especially the nose and chin, with superimposed small pigmented papules scattered on normal and hyperpigmented facial skin (Figures 1,2). She had no discoloration on other body sites such as the neck, hands or trunk and there was no evidence of arthritis or joint pain. The complete blood cell count, urine analysis and creatinin levels were normal.

Background and aim:Basal cell carcinoma (BCC) is the most common cancer in whites. The most important risk factor for developing BCC is solar ultraviolet (UV) irradiation. As the weather in most parts of Iran is mostly sunny, BCC is common. This study was performed to evaluate changes in the frequency of BCC in 1368-69 and 1378-79.Materials and Methods: This descriptive and retrospective study was performed as an existing data study and all files in department of pathology in Razi Hospital from 1368-69 and 1378-79 were assessed with regards of BCC frequency, patient age and gender as well as history of radiation and the collected data were compared.Results: A total of 1503 BCCs were studied, 507 were from 1368-69 and 996 from 1378-79 period. In the years 1368-69, relative frequency of BCC was 11.1%. On the other hand in years 1378-79 relative frequency of BCC was increased to 15.2%, which was not associated with gender. BCC occurance showed a significant decrease in 20-30-year age group and a significant increase in over 60 years age group. Among histologic subtypes, nodular form showed an increase, which was statistically significant on face region (P<0.005), but was not linked to patient gender. In male patients significant decrease of BCC frequency on face and increase on head and neck areas was observed, but there was not any significant change in tumor location in female patients.Conclusion:During a 10-year period the frequency of BCC has increased significantly requiring more attention to develop educational programs regarding prevention, diagnosis and early treatment.

A 77-year-old Iranian man was visited at the Dermatology Department of Loghman-e Hakim Hospital affiliated with Shahid Beheshti University of Medical Sciences on June 2012 with bilateral, symmetric, raised, violaceous and painless plaques on the nose (both ala nasi), from one year ago. The lesions were small at first and then grew gradually larger in size. Systemic symptoms such as fever, weight loss and night sweat were absent. He had no history of systemic disease, bite, trauma, vaccination, or medication. Past medical history was negative except for cataract in several years ago and he was in good health. He had a history of treatment with oral antibiotics without any improvement. Physical examination revealed bilateral, symmetric, indurated, violaceous, non-tender plaques on the nose (both ala nasi) with some scales and follicular accentuation without discharge (Figure 1). Other examinations were normal and laboratory tests revealed no abnormality. A skin biopsy was performed.

Chemical leucoderma is an acquired hypopigmented dermatosis caused by repeated exposure to specific chemical compounds, notably the aromatic or aliphatic derivatives of phenols andcatechols which are commonly present in the rubber and plastic materials. Herein, we report a case of chemical leucoderma in a 55-year-old male, appearing bilaterally over the retroauricular areas, which appeared after of the use of a new spectacle frame by the patient. The patch test results with Indian Standard Series and Cosmetic Series were negative. Chemical leucoderma following the use of materials like hair dyes, and rubber shoes or gloves is common but only a few cases of leucoderma with spectacles have been reported in the literature to date.
